Population Health and connecting it to your data privacy pages
The Whole System Integrated Care Dashboard is a population health tool used by your GP practice to help understand health trends and plan care across the patient population. It does not use artificial intelligence, and it is not designed to make clinical decisions or give medical advice. It is not considered a medical device. Sometimes, when you look at your record, you may see codes such as “frailty”. These codes are used by your care team to help identify patients who may benefit from extra support or proactive care. Seeing a code in your record does not mean any immediate change to your care—it is part of how your practice plans and coordinates care. If you have questions about what this means for you, you can always speak with your GP or care team.
