Population Health and connecting it to your data privacy pages
The Whole System Integrated Care Dashboard is a population health tool used by your GP practice to help understand health trends and plan care across the patient population. Seeing a code in your record does not mean any immediate change to your care—it is part of how your practice plans and coordinates care.
