Come to the practice between 8am and 6:30pm (Monday to Friday) if you would prefer to collect and complete a paper registration form in person.

Frequently asked questions about registering with an NHS doctor

What information should I have on hand when registering?

You will be asked for basic details such as your name, date of birth, address and contact information only. Having your NHS number can help us find your records faster, but you do not need an NHS number, proof of address, ID or proof of immigration status to register. You may include the patient’s medical history if available, however, this is optional. 

Can I register even if I do not need medical treatment right now?

Yes. Registering in advance means you are already set up with a GP practice if you need advice, prescriptions, referrals, routine care or help unexpectedly in future. It also means faster access to treatment when you do need it. 

Can I register with Grand Union Health Centre even if there are closer NHS GP practices to my home?

Yes. While many people choose to register with the closest GP surgery, you are free to register with any NHS GP that you are comfortable travelling to. Our practice is just a short walk from Queensway, and easily accessible by bus. 

Do I need a photo ID, proof of address, immigration documents, or an NHS number to register?

No. You can complete the registration without a photo ID, proof of address, immigration documents, or NHS number. 

Can I register if I’m visiting the UK or living here temporarily?

Yes. Anyone can register with Grand Union Health Centre and receive free primary care, including GP consultations and treatment. However, if you need care outside the practice, such as hospital treatment or some other NHS services, charges may apply if you are a visitor or temporary resident.

Registering with our GP practice is free for everyone. If you need hospital or specialist care, separate NHS charging rules may apply depending on your status.

How can I register with Grand Union Health Centre?

You can register with Grand Union Health Centre in whichever way is easiest for you:

  • Online: complete the online registration form via the NHS registration service
  • By email: download the registration form PDF, complete it at home, and email it back to the practice
  • In person: download and complete the form, print and complete it at home, then bring it to the practice yourself
  • By delivery or courier: download the form, print and complete it at home, and have it delivered to the practice
  • At the practice: visit us and ask for a paper registration form to complete

To help us process your registration as quickly as possible, please make sure the form is completed as fully as you can. Make sure to sign and date the registration form.

Is registering with an NHS DR free?

Yes. Registering with any NHS doctor, surgery, or practice is free nationwide.

What happens after registration?

Once your registration is complete:

  • You will be assigned a GP
  • You will be send a message saying you are registered and ask you to download the NHS app
  • You can then book appointments on the NHS app
  • If you already have the NHS app make sure you update it so you can continue accessing your medication and medical records

Temporary Services

If you feel unwell away from home or need a doctor but aren't registered, you can register as a temporary patient. This lasts for up to 3 months. It allows you to join the local practice list while staying a patient of your permanent GP. After three months, you must re-register as a temporary patient or register permanently with that practice.
